Aftercare Support Specialist

We are seeking a skilled professional to fill the role of Aftercare Support Specialist.

The successful candidate will be responsible for delivering health education to clients and the community, participating in the Aftercare Services Community of Practice, and providing awareness and aftercare prevention programs.

The ideal candidate will possess qualifications in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care, as well as a passion for supporting the Aboriginal and Torres Strait Islander community.

The role involves working with a localised service delivery model for aftercare services through a co-design approach with the local community.

Key Responsibilities:
  • Deliver health education to clients and the community
  • Participate in the Aftercare Services Community of Practice
  • Provide awareness and aftercare prevention programs
  • Develop community work projects and programs designed to enhance the community's ability to respond to the needs of Aboriginal people
  • Build and maintain strong referral pathways into relevant services
  • Provide community health education and promotion activities and develop, organise and run programs
  • Improve Aboriginal and Torres Strait Islander mental health and suicide prevention outcomes by providing culturally appropriate aftercare service and support
Requirements:
  • Qualifications in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care OR equivalent training or minimum two years' experience within the field
  • A passion for supporting the Aboriginal and Torres Strait Islander community
  • Demonstrated understanding of relevant legislation and policy frameworks
  • Driver's licence and willingness to complete relevant Police and WWC checks
