Care Manager Professional

The role of a Care Manager Professional is to deliver exceptional care to residents, promoting individualised support that enhances and promotes their quality of life.

As a senior member of the operations leadership team, you will oversee a team of care staff, empowering them through learning, support, and leadership to advance the level of service to your residents.

You will work in conjunction with your General Manager to ensure care governance and quality compliance are achieved through robust processes that meet internal and external auditing standards.

Key Responsibilities:

  • Sustain and grow services through diligence with funding, care planning, and community engagement
  • Lead all clinical services in line with operational process and policy
  • Deliver Quality Care Services and professional practice
  • Oversee and encourage continuous improvement activities in all aspects of clinical and lifestyle services
  • Ensure accurate and timely completion of clinical documentation
  • Coordinate and support scheduled audits and clinical indicators
  • Maintain up-to-date resident health records and clinical documentation
  • Monitor best practice medication management guidelines

About You

To be successful in this role, you will possess:

  • Relevant tertiary qualifications or Australian recognised equivalency
  • Current registration with AHPRA as a registered nurse
  • Senior clinical care experience in the health sector with clinical experience in aged care
  • Demonstrated leadership and influencing skills
  • Sound understanding of care assessment in the Aged Care Sector and use of AN-ACC
  • Effective interpersonal and communication skills (written and verbal)
