Vivir Healthcare is seeking a skilled Dietitian to join our Community Home Care team in Melbourne North. As a Dietitian, you will play a vital role in providing nutrition and dietary services to clients funded by various programs, including HCP, CHSP, and STRC.
Key Responsibilities- Assess and address the nutrition and dietary needs of clients with diverse disabilities and impairments.
- Develop and implement individualized treatment plans to address deficits in nutrition and dietary areas.
- Communicate effectively with clients and funding organizations to ensure seamless service delivery.
- Member of the Dietitian Association of Australia (DAA) with Provisional or Full APD status.
- Valid drivers' license and reliable vehicle.
- Professional indemnity insurance or willingness to obtain.
- Valid NDIS Worker Screening Clearance or willingness to obtain.
- 2023 Flu Vaccine Certificate or willingness to obtain.
- COVID vaccine certificate or willingness to obtain.
