Caregiver Coordinator

We are seeking a qualified Community Support Worker to join our team at Allambi Care. As a Community Support Worker, you will play a critical role in changing the lives of young people in our community.

You will work in a variety of settings, including participants' homes, schools, hospitals, and community centers, promoting their goals for independence and supporting them in their daily lives.

• Provide support and guidance to participants in their daily lives, including assistance with personal care, household tasks, and social activities.
• Develop and implement individual support plans, working closely with participants, their families, and other stakeholders to achieve their goals.
• Conduct risk assessments and implement strategies to ensure the safety and well-being of participants.
• Collaborate with other professionals, including social workers, psychologists, and healthcare providers, to ensure comprehensive support for participants.
• Maintain accurate and timely records of participant interactions, progress, and outcomes.

• Bachelor's degree in a relevant field, such as social work, psychology, or a related discipline.
• Current First Aid Certificate and Working with Children's Check Clearance.
• Comprehensive insurance and a reliable vehicle for transporting participants.
• High level of assessment skills, information gathering, problem-solving, and evaluation skills.
• Ability to work in a non-judgmental manner and provide support for children, young people, and program participants.
