Advanced Clinical Practitioner in Gerontological Nursing

We are seeking a skilled Aged Care Nurse Practitioner to fill this challenging and rewarding role.
Key Responsibilities:- Deliver expert clinical assessment, diagnosis, treatment, and management to older patients in the ED and community settings.
- Practice autonomously, initiating and managing patient care, and providing evidence-based interventions.
- Lead preventative health programs and educate patients in various settings.
- Collaborate with general practitioners and specialist medical officers to guide patient treatment.
- Must meet NMBA competencies for Registered Nurses and hold current registration with AHPRA as a Nurse Practitioner, with at least 5 years' experience in Aged Care.
- Be able to work independently and within a multidisciplinary team across aged care, acute, and community settings.
- Possess highly developed communication skills, including written, verbal, and interpersonal abilities, with expertise in computer literacy and database programs.
- Demonstrate advanced nursing skills in Aged Care and Dementia, with extensive experience in caring for patients with complex medical, cognitive, and social needs.
- Show leadership capabilities, along with expertise in teaching, education, and research related to Aged Care and Dementia.
- Hold a valid Class C drivers licence and be willing to travel for work purposes.
