Main Responsibilities
- Provide expert care to patients with life-limiting illnesses, working closely with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences.
- Collaborate with teams to develop and implement care plans that meet individual patient needs.
- Offer health education and support to patients, families, and care staff to ensure optimal outcomes and understanding of care processes.
- Build strong partnerships with community services and families to manage patient care effectively.
Requirements
- Current endorsement with the Nursing and Midwifery Board of Australia via AHPRA as a Registered Nurse.
- Palliative care experience.
- Postgraduate qualification in Palliative Care preferred/working towards.
- Advanced interpersonal, communication (written and verbal) and problem-solving skills.
