Expert Medication Manager

Job Description:
The Clinical Pharmacist will be responsible for providing comprehensive pharmacy services to patients and clients of the health district.
Key Responsibilities:
- Provide clinical pharmacy services in accordance with NSW Health policies and procedures, ensuring high-quality patient care and adherence to professional standards.
- Collaborate effectively with multidisciplinary teams to deliver exceptional patient outcomes, foster a culture of teamwork and communication, and promote cross-functional collaboration.
- Develop and implement evidence-based clinical pharmacy programs that enhance patient care, improve health outcomes, and align with the organization's strategic objectives.
- Conduct regular medication reviews, assess treatment plans, and provide recommendations for optimization to ensure optimal patient care and safety.
Required Skills and Qualifications:
- Current registration or eligibility for registration as a Pharmacist with the Australian Health Practitioners Regulation Agency (AHPRA).
- Demonstrated experience and skills in clinical pharmacy, preferably in the hospital setting, with a strong emphasis on patient-centered care and quality improvement initiatives.
- Effective written, verbal, and interpersonal skills, with the ability to communicate complex information to diverse audiences.
- Ability to work collaboratively within a multidisciplinary team environment, prioritize tasks, and manage multiple projects simultaneously.
Working Conditions:
- May require driving long distances as part of job requirements.
- Must be able to maintain an unrestricted driver's license.
