About The Role

Key Responsibilities
  • Conduct thorough functional assessments to identify client needs and develop personalized plans.
  • Deliver occupational therapy services in clients' homes and community settings, promoting independence and empowerment.
  • Collaborate with multidisciplinary teams, including medical and allied health professionals, to ensure comprehensive care.
  • Develop and implement client plans, ensuring effective monitoring and reviews.
  • Conduct home and community visits as required, providing support and guidance to clients and their families.
  • Liaise with medical and allied health professionals to ensure seamless care coordination.
  • Coach and advise team members and carers on occupational therapy best practices.
