We are seeking a skilled Physiotherapist to join our dynamic team, working in community aged care on a full-time or part-time permanent basis. This rewarding role involves assessing, planning and delivering individualized or group physiotherapy services and activities to improve client well-being in their homes.
About the RoleThis is an opportunity to make a meaningful difference in the lives of older people, enabling them to age well and maintain independence. As a Physiotherapist, you will work as part of a multidisciplinary team to assess clients' physical and functional abilities, develop and deliver individualized treatment plans, and implement evidence-based interventions addressing mobility issues, pain management, and age-related conditions.
Key Responsibilities- Conduct comprehensive assessments of clients' physical and functional abilities to understand their needs.
- Develop and deliver individualized or group treatment plans based on clients' needs, goals and preferences.
- Implement evidence-based physiotherapy interventions addressing mobility issues, pain management, and age-related conditions.
- Regularly assess and report on clients' physical and psychosocial capabilities to the care management team.
- Maintain accurate records, exercise sheets, assessment reports and progress notes reflecting clients' status.
- Make recommendations regarding exercise programs, mobility and safety at home; refer to other disciplines as required.
- Uphold ethical and professional standards per regulatory guidelines.
- Collaborate with interdisciplinary teams (care advisors, occupational therapists, fitness instructors, allied health assistants, etc.).
- Educate clients and families on preventive measures and exercises to improve and maintain mobility.
- Participate in team meetings, case conferences and training sessions to improve care delivery.
- Engage with team members, clients and families respectfully and support a culturally diverse team.
- Build relationships with clients and their families, considering challenges in the aged care landscape.
- Bachelor or Postgraduate Degree in Physiotherapy and current registration with relevant authorities.
- Excellent interpersonal, verbal and written communication skills.
- Ability to manage all aspects of compliance.
- Experience working within multidisciplinary teams.
- Effective time management and ability to prioritise and organise own work/caseload.
- Demonstrated computer literacy.
- Knowledge and experience of health, aged and/or disability services.
- Current driver's licence and access to a registered and insured motor vehicle.
- Paid travel time and kilometres reimbursed.
