About the Role

We are seeking an experienced Occupational Therapist to join our multidisciplinary team at Sunbury and Cobaw Community Health. As an Occupational Therapist, you will play a vital role in delivering high-quality services to our local community, working with individuals and families to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Complete goal-directed care planning, case coordination, equipment and home modification prescription, and chronic illness management.
  • Develop and implement individualized treatment plans to promote independence and participation in daily activities.
  • Collaborate with other healthcare professionals to provide comprehensive care and support to clients.
  • Conduct assessments and evaluations to identify client needs and develop strategies to address them.
  • Provide education and support to clients and their families to promote healthy behaviors and lifestyle choices.
Requirements
  • At least two years of clinical occupational therapy experience.
  • Excellent interpersonal skills and the ability to communicate effectively with clients, families, and carers as well as other service providers.
  • Knowledge and experience working with My Aged Care, State Wide Equipment Program and Geat 2 Go.
  • Knowledge and experience working under fee for service models such as the NDIS and/or Home Care Packages.
