Key Responsibilities- Provide skilled nursing care to clients in their homes, including wound management, continence management, diabetes management, medication management, patient assessment, and documentation of clinical care.
- Assess client's well-being and homecare needs, provide direct care to the client, make recommendations about care enhancements, and monitor for changes in the client's health status.
- Collaborate closely with the rest of the care team, including a registered nurse, care manager, and other multidisciplinary team members.
- Develop and implement individualized care plans that meet the unique needs of each client.
- Provide education and support to clients and their families on health-related topics and disease management.
- Current registration as a Registered Nurse with a valid nursing license.
- Minimum 2 years of experience in homecare or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
