Family Doctor is seeking an experienced Registered Nurse to join our team as a Chronic Disease Management Nurse. This is an exciting opportunity for a skilled nurse to provide expert care and training to nurses in general practice, while supporting the learning and development of all staff.
The successful candidate will play a key role in achieving the successful completion of chronic disease management plans, reviews, and health assessments. They will provide high-quality clinical training and mentorship around chronic disease management in a general practice setting, adhering to best practice standards.
Key Responsibilities- Chronic Disease Management Planning: Successfully complete GP Management Plans, Team Care Arrangements, and Health Assessments within agreed timeframes.
- Practice Support: Assist practices in identifying appropriate patients and maintaining a recall system for reviews.
- Patient Assessment and Documentation: Assess patients, document results, and identify needs.
- Preventive Health and Aged-Based Immunization: Understand and implement preventive health and aged-based immunization requirements.
- Referral and Review: Assist in referring patients to allied health services and provide ongoing assistance with reviews and re-assessment of patients.
- Medicare Item Numbers and Claims: Ensure accurate claiming of Medicare item numbers for CDM and HA item numbers.
- Mentorship and Training: Model and mentor CDM processes to other nurses within Family Doctor.
- Current AHPRA Registration - Registered Nurse (Division 1) General.
- Background working in a General Practice and/or primary health setting.
- Experience in Chronic Disease Management, Health Assessments, and an understanding of their utilization in the CVC Program.
- Experience with Best Practice (an advantage).
- Experience with MyMedicare.
- Capacity to travel to regional Victoria.
- Regular training and education.
