Team Leader NDIS SIL

We are seeking a highly skilled and experienced Team Leader to join our NDIS Services team in Muswellbrook. As a Team Leader, you will play a crucial role in ensuring quality supports that foster choice, independence, and goal achievement for participants.
Key Responsibilities- Drive Samaritans' Person-Centred Planning processes, overseeing support delivery, maintaining up-to-date documentation, and providing a consistent point of contact for supported individuals and their families.
- Assist in identifying and assessing client needs, supervise staff, ensure professional and compassionate service delivery, and manage various personal care tasks.
- Maintain effective communication and relationships with stakeholders, supports clients' living skills development, ensures accurate and timely documentation, reports incidents, and contributes positively to teamwork and achieving outcomes.
- Tertiary qualification in a relevant field or commensurate experience.
- Demonstrated experience in the delivery of services to people with a disability.
- Established coordination and problem-solving skills.
- Demonstrated knowledge and understanding of NDIA planning processes.
- Ability to think creatively and use initiative, to work independently or as part of a multidisciplinary team.
- Ability to effectively develop and maintain positive working relationships and networks with government and non-government agencies and services.
- High level interpersonal skills that support positive and productive relationships.
- Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents.
