- Collaborate with the Physical Therapist to identify target lists, including short-term and long-term goals.
- Conduct individual and group treatment sessions, including exercises and activities to improve muscle strength, coordination, mobility, and sensory awareness.
- Monitor resident progress and consult with the Physical Therapist to modify treatment plans as needed.
- Participate in resident, family, and staff education to promote a culture of excellence in care.
- Adhere to department and resident schedules, and modify as necessary to achieve maximum productivity and efficiency.
- Assist with quality assurance programs and maintain accurate and compliant documentation in electronic medical records.
- Associate's degree in Physical Therapy from an accredited program.
- Active Physical Therapy Assistant license in the state(s) of practice.
- Ongoing continuing education and professional development to maintain current licensure and certification.
