About the Role

As a Support Coordinator, you will be responsible for providing support to National Disability Insurance Scheme (NDIS) participants. Your role will involve assisting participants in implementing their NDIS plans and therapies, helping with home modifications and Assistive Technology applications, maximising participants' NDIS plans and engaging in mainstream supports.

Key Responsibilities:
  • Assist participants in developing and implementing their NDIS plans
  • Help participants access and utilise available support services
  • Maintain accurate records of participant interactions and progress
  • Collaborate with other stakeholders to ensure seamless service delivery
Requirements:
  • Relevant experience in Support Coordination or transferable skills
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Excellent organisational and time management skills
  • A current Driver's Licence and ability to travel
  • Police check within the last 6 months and NDIS Worker Screening
