Job Overview

We are seeking experienced professionals to fill the role of Advanced Health Practitioner.

About the Role:

  • Responsibility for assessing patient needs and determining appropriate care pathways.
  • Act as an autonomous practitioner, taking ownership of patient assessments, treatment plans, and referrals as required.
  • Conduct comprehensive patient histories, physical examinations, and use expert clinical judgement to identify potential diagnoses.
  • Independently prescribe medications and make onward referrals to specialist services as necessary.
  • Ensure continuity of care for patients.

Key Requirements

  • A proven track record of delivering high-quality care within Primary Care settings.
  • Experience in managing a wide range of minor ailments.
