Job Title: Disability Support Worker

We are seeking experienced Disability Support Workers to join our team, working across various locations in Melbourne. As a key member of our team, you will deliver high-quality care services to individuals with diverse needs, including Autism/ADHD, Mental Health, Diabetes, Epilepsy, Intellectual Disabilities, and Dementia.

With a focus on client-centered support, you will work collaboratively with our clients, their families, and other healthcare professionals to create personalized care plans that promote independence, dignity, and well-being.

Responsibilities
  • Provide person-centered support to individuals with disabilities, promoting their autonomy and self-advocacy.
  • Develop and implement individualized care plans in consultation with clients, their families, and multidisciplinary teams.
  • Deliver practical assistance with daily living activities, such as personal care, meal preparation, and household management.
  • Support clients with complex behaviors, using de-escalation techniques and positive behavior support strategies.
  • Work collaboratively with other service providers, including allied health professionals, to ensure comprehensive care and support.
Requirements
  • A minimum of 2+ years' experience working as a Disability Support Worker.
  • Experience working with people with Autism/ADHD, Mental Health, Diabetes, Epilepsy, Intellectual Disabilities, Mental Health, Dementia, and Advance Manual Handling Supports highly regarded.
  • Current NDIS Worker Screening Check and Working with Children Check required.
  • A current Police Check, Vaccinations (including COVID), CPR, and First Aid certification essential.
  • Driving Licence and access to a reliable vehicle necessary for transportation to clients' homes.
  • Cert III Individual Supports, Cert IV Disability, and/or Mental Health Care Qualification preferred.
Salary and Benefits

We offer a competitive salary range of $55,000 - $65,000 per annum, depending on experience, plus superannuation and paid annual leave. Other benefits include:
