We are seeking a highly skilled and experienced Chronic Disease Management (CDM) Nurse to join our team at ForHealth Medical Centres. As a CDM Nurse, you will play a vital role in supporting our patients to manage their chronic conditions and improve their overall health and wellbeing.
Key Responsibilities- Develop and implement care plans to help patients manage their chronic conditions
- Conduct health assessments and identify early signs of chronic disease
- Work closely with GPs and other healthcare professionals to promote preventative healthcare
- Empower patients to take ownership of their health and make informed decisions
- Provide education and support to patients on healthy lifestyle choices and disease management
- AHPRA registered Enrolled or Registered Nurse
- Proven experience in Chronic Disease Management and health assessments
- Strong organisational and time management skills
- Outstanding communication and customer service skills
- A passion for people and a personable approach
