We are seeking a Community Care Lead to join our team, who will play a crucial role in supporting our Community teams and enabling our clients to live their best possible life. This position offers opportunities for career progression and the chance to work with a talented team of professionals who share your passion for care and support.
As a Community Care Lead, you will be responsible for supervising, mentoring and training a team of PCWs and CSA's to optimise support of our clients. You will also provide ongoing support to our team, including backfilling roster shortfalls, and liaise with our community clients to monitor their service and meet care plan requirements. Additionally, you will pro-actively participate and communicate with the Community HCP team to optimise service delivery.
To be successful in this role, you will need to have at least two year's paid experience as a PCW or EN working in the Community and be a current team member. You will also require an empathetic, patient and caring nature, a motivation to support junior colleagues, flexibility to prioritise client requirements, and availability to work minimum 25 hours per week, Monday – Friday roster. Exceptional verbal and written communication skills are essential for this role.
