The Role

This position involves assessing and managing clients impacted by mental health issues who have been referred to the Social Health Commonwealth Psychosocial Support Program (CPSP). You will operate within a strengths-based framework, working closely with families and individuals to identify their unique needs and develop tailored support plans.

You will be responsible for providing goal-driven case management support, focusing on building resilience and capacity among clients. This may involve conducting intakes, maintaining regular contact, data management, responding to critical incidents, and delivering direct supported care to facilitate access to holistic health and wellbeing services.

Key Responsibilities
  • Conduct thorough intake assessments with suitable program participants
  • Maintain regular communication with participants via preferred channels (e.g., face-to-face, phone, Zoom/Teams)
  • Accurately record and manage participant data
  • Respond effectively to critical incidents, providing well-being first aid as needed
  • Provide direct supported care to facilitate patient access to holistic health and wellbeing services

Requirements

To succeed in this role, you will require:

  • Tertiary qualification in Social Work or Human Services (or related field) and/or demonstrated experience in supporting individuals with complex needs
  • Demonstrated ability to apply high-level practice judgment when working with clients to set and achieve goals
  • Capacity to work effectively with culturally and socially diverse groups, applying a culturally responsive framework to guide interventions
  • Ability to communicate clearly and collaborate as part of a multidisciplinary team
  • Demonstrated IT capability, including experience with computer-based data management systems and Microsoft Office
  • A willingness to travel across the ATSICHS Brisbane region and work across different sites
  • A valid Working with Children's Card (Blue Card) and NDIS Worker Screening (Yellow Card), or eligibility to obtain
  • National Police Check or eligibility to apply
Preferred Qualifications

Previous experience in a multi-disciplinary team environment and/or working in an Aboriginal Medical Service would be highly regarded.
