Job Description

The role of the NDIS Facilitator is a crucial one, ensuring that individualized services are developed and delivered to customers.

This involves developing, maintaining, and reviewing individualized support plans in consultation with the customer, their family, guardians, and key health professionals.

  • Oversee the day-to-day running of the NDIS team, including service agreements, care plans, documentation, and compliance.
  • Manage and oversee SIL/SDA services, including new participant intake, sourcing new housing, and completing submissions.
  • Supervise and manage SIL staff, including orientation, performance management, and probation reviews.
  • Ensure accurate billing for each individual SIL home.
  • Liaise with participants' teams to ensure smooth transitions into SIL.
  • Ensure strict compliance with NDIS approvals and verify all services, including transport costs and billing.
  • Assist with billing issues for NDIS participants as needed.
Required Skills and Qualifications
  • Qualification in Community Services, Human Services, Nursing, or equivalent.
  • Current and clear NDIS worker screening.
  • Previous experience working in NDIS and SIL/SDA housing roles.
  • Supervising, supporting, and training staff experience.
  • Ability to work flexibly in response to changing customer needs.
  • Capacity to coordinate services according to program guides.
  • Experience working within multi-disciplinary teams.
  • Ability to develop and maintain supportive networks with specialty services.
  • Highly developed interpersonal skills and ability to build effective relationships.
  • Excellent verbal and written communication skills.
  • Strong organisational and time management skills.
  • Ability to work as part of a team.
  • Sound computer skills.
  • Current driver's licence.
