We are seeking a highly skilled and experienced Dementia Consultant to join our team at Hammond Care. As a Dementia Consultant, you will play a critical role in delivering Australian Government funded dementia behaviour support services.
As a Behaviour Support Specialist, you will be responsible for supporting families, caregivers, and clinical staff to understand how dementia impacts each individual. You will equip carers and clinicians to prevent and respond to behaviour changes through modelling, coaching, and development of resources including behaviour support planning.
Key Responsibilities:
- Support families and caregivers to understand dementia and its impact on individuals
- Develop and implement behaviour support plans to prevent and respond to behaviour changes
- Conduct assessments to understand the holistic experience of individuals and their care networks
- Work in partnership with the broader Hammond Care team to build capability across the home and aged care sectors
- Contribute to research, projects, and programs to meet the changing needs of the sector
Requirements:
- Expertise in supporting people living with dementia
- Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health
- Experience in Australian residential aged care and/or community aged care
- High level of negotiation, communication, and relationship building skills
- Excellent problem-solving and decision-making skills
- Ability to prioritise multiple tasks and meet deadlines
- Excellent written and verbal communication skills
- Empathy and patience
