We are seeking an experienced Registered Nurse to join our team as a Home Care Specialist.

Key Responsibilities
  • Provide high-quality patient care in home settings, including assessment, planning, implementation, and evaluation of care.
  • Work collaboratively with other healthcare professionals to develop and implement patient care plans.
  • Provide education and support to patients and their families on health conditions, treatment options, and self-care techniques.
  • Conduct regular assessments of patients' physical and emotional needs, and update care plans accordingly.
Requirements
  • Currentrnursing registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Valid NSW driver's licence.
  • Full working rights within Australia.
