About the Role

We are seeking an experienced Advanced Clinical Pharmacist to join our team at Primary Care 24 (PC24), a leading social enterprise delivering NHS contracts exclusively. As a key member of our organisation, you will play a vital role in developing and delivering our medicines optimisation and medicines safety agenda.

Key Responsibilities
  • Medicines Optimisation: Develop and implement strategies to improve the safe and effective use of medicines across our practices.
  • Medicines Safety: Contribute to the management of long-term conditions, repeat prescribing, clinical medication reviews, and clinical assessment and diagnosis within your sphere of competency.
  • Leadership and Collaboration: Work closely with our clinical team to develop and implement prescribing policies, and collaborate with care home staff to improve the safety of medicines ordering and administration.
  • Clinical Expertise: Provide clinical leadership and expertise to our team, including undertaking clinical medication reviews, repeat prescribing, and seeing patients where medicine optimisation is required.
Requirements
  • Qualifications: Degree in Pharmacy and Independent Prescriber V300 qualification.
  • Experience: Minimum of 2 years post-qualification experience in urgent care and/or primary care, with in-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare.
  • Skills: Ability to work at pace in a busy working environment, multitask, and provide clinical supervision.
What We Offer
  • Competitive Salary: £50,952 to 58,439 per annum DOE, based on a 37.5 hour week.
