About the Opportunity

We are seeking a Support Worker to join our Community Services Team at Southern Cross Care Queensland.

The successful candidate will work closely with our Care Partners to deliver a personalised support service to our clients, focusing on their health and independence.

Key Responsibilities and Requirements

  • Deliver essential services such as personal care, meal preparation, and social support.
  • Maintain a safe and clean-living environment for clients.
  • Accompany clients to medical appointments or on social outings.
  • Monitor and report changes in clients' health or behavior to supervisors or healthcare professionals.
  • Maintain accurate documentation of the care provided in accordance with SCCQ standards.
  • Certificate III in Disability, Aged Care, Individual Support, Home & Community Care or equivalent.
  • Exceptional people & interpersonal skills.
  • Ability to work autonomously and to manage your time effectively.
  • A current Australian Drivers Licence.
  • Own reliable car to use for work with full comprehensive car insurance and current registration.
