We are seeking an experienced Aboriginal and Torres Strait Islander Health Practitioner to join our team at Gold Coast Health, where we deliver comprehensive and compassionate world-class care to address specialist palliative care needs for consumers across the region.
As a key member of our Supportive and Specialist Palliative Care Service, you will work collaboratively with other healthcare professionals to provide a culturally responsive service that meets the unique needs of our Indigenous community.
Key Responsibilities- Provide specialist palliative care services to Indigenous patients and their families, working closely with other healthcare professionals to ensure seamless care.
- Develop and implement culturally responsive care plans that address the physical, emotional, and spiritual needs of patients.
- Engage with community members to strengthen community involvement in health service delivery and promote health literacy.
- Conduct community home visits to provide support and education to patients and their families.
- Lead quality improvement activities to improve clinical practice and patient outcomes.
- Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
- Possession of Diploma in Aboriginal and/or Torres Strait Islander Primary Health or equivalent.
- ATSIHPBA Registration.
- Drivers Licence.
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B.
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required.
- Declaration of serious discipline history must be disclosed.
