Disability Support Coordinator

A Disability Support Coordinator with Activ in the Upper South West region empowers individuals with disabilities to reach their full potential through personalised support and community engagement.
We are currently seeking expressions of interest for vacancies in the area. Suitable candidates may be added to the talent pool and contacted directly for future opportunities, working across the Upper South West region from Australind to Usher.
- Permanent part-time hours
- Casual hours, anticipating 15-25 hours per week for leave coverage, including shifts starting at 6am, 8pm, and overnight sleep shifts.
A typical day may involve:
- Supporting person-centred plans to achieve life goals
- Engaging customers in recreational activities like swimming, dancing, bingo, horseback riding, and bowling
- Assisting with daily living skills, such as meal preparation, housekeeping, showering, toileting, medication administration, and attending appointments
- E nsuring positive behaviour support plans are followed
- Administrative duties and mandatory daily reports
To succeed in this role, you will need:
- Live in the Upper South West region
- Be willing to support customers with personal care and domestic duties
- Have well-developed interpersonal skills and ability to build effective working relationships
- Passion for providing service and assistance, and helping others live their best lives
- Previous experience or willingness to train in manual handling and medication administration
- Proven administration and computer literacy skills
- Availability to work between 15-25 hours per week
- Cert III in Individual Support (CHC33015 or CHC33021), or willingness to work towards the qualification
- A valid driver's licence and a willingness to use your private vehicle (with rebates available)
