Disability Support Coordinator

A Disability Support Coordinator with Activ in the Upper South West region empowers individuals with disabilities to reach their full potential through personalised support and community engagement.

  • Permanent part-time hours
  • Casual hours, anticipating 15-25 hours per week for leave coverage, including shifts starting at 6am, 8pm, and overnight sleep shifts.
About the Role

A typical day may involve:

  • Supporting person-centred plans to achieve life goals
  • Engaging customers in recreational activities like swimming, dancing, bingo, horseback riding, and bowling
  • Assisting with daily living skills, such as meal preparation, housekeeping, showering, toileting, medication administration, and attending appointments
  • E nsuring positive behaviour support plans are followed
  • Administrative duties and mandatory daily reports

To succeed in this role, you will need:

  • Live in the Upper South West region
  • Be willing to support customers with personal care and domestic duties
  • Have well-developed interpersonal skills and ability to build effective working relationships
  • Passion for providing service and assistance, and helping others live their best lives
  • Previous experience or willingness to train in manual handling and medication administration
  • Proven administration and computer literacy skills
  • Availability to work between 15-25 hours per week
  • Cert III in Individual Support (CHC33015 or CHC33021), or willingness to work towards the qualification
  • A valid driver's licence and a willingness to use your private vehicle (with rebates available)
