Diabetes Educator

We are seeking a highly skilled and dedicated Registered Nurse - Diabetes Educator to join our team at Anyinginyi Health Aboriginal Corporation. As a key member of our Health Services Section, you will play a vital role in improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions.
Key Responsibilities- Contribute to improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care.
- Assist Aboriginal and Torres Strait Islander people to understand diabetes and how to manage it.
- Provide high-quality diabetes education to individuals with diabetes, their families, and support networks.
- Work closely with GPs and other health professionals to ensure that services offered to clients meet contemporary standards for high-quality care and consistency with diabetes treatment and management.
- Provide high-quality diabetes nursing care to individuals with diabetes, their families, and support networks.
To be considered for this position, you must be a Registered Nurse with current AHPRA Registration, with a Graduate Certificate in Diabetes Education and/or relevant experience providing clinical care and education to individuals with diabetes. Experience working with an Aboriginal organisation or communities will be highly regarded, but is not essential. You must be able to demonstrate a clear understanding of the issues pertaining to Aboriginal Primary Health Care and the social determinants of health in our surrounding communities.
