This is an exciting opportunity for a Registered Nurse to join our team as a Case Manager in the Casey Area Mental Health Service. As a key member of our team, you will be responsible for providing high-quality care to patients with complex mental health needs.

As a Case Manager, you will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans. You will also participate in daily team functions, including the duty role, multi-agency liaison, and crisis management.

We are looking for a skilled and compassionate nurse who is passionate about delivering patient-centered care. If you have a strong background in mental health nursing and are eager to make a difference in the lives of our patients, we encourage you to apply.
