Care Coordinator for Individuals with Complex Needs

We are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing personal care, assistance with daily activities, and emotional support to individuals living with disabilities.
About the RoleThis is an exciting opportunity to work with a supportive and collaborative team at Programmed Health Professionals. As a Care Coordinator, you will have the chance to make a real difference in the lives of people with complex needs.
Duties and Responsibilities- Provide personal care and assistance with daily activities to individuals living with disabilities.
- Help clients achieve their personal goals and independence.
- Assist in social, recreational, and community participation activities.
- Manage challenging behaviors and provide tailored support based on individual care plans.
- Maintain a safe and supportive environment for clients.
- Current Certificate III or IV in Aged Care, Individual Support, Disability, or Community Care.
- Experience working with clients who have complex needs in an Aged Care environment.
- Valid driver's license with a reliable vehicle and current car insurance.
- Current First Aid and CPR Certificate.
- NDIS Workers Clearance (or willingness to obtain).
- Flexibility to work various shifts, including weekends and public holidays.
- Able to administer medication.
- A competitive estimated salary of $65,000 - $80,000 per annum, paid weekly.
- Immediate start with full orientation provided.
