The Heart of the Role

This role is pivotal in achieving exceptional outcomes for our residents by working under the guidance of allied health professionals to support the implementation of care plans.

  • Host engaging group and individual programs, therapy, and exercise sessions
  • Support telehealth assessments to provide timely interventions
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Document progress notes and charts to track resident advancements

You will be part of a supportive and collaborative team committed to enhancing the well-being of our residents and participating in continuous improvement activities within the care home.

To succeed in this role, you will need:

  • Minimum Certificate III qualification in Ageing, Individual Support, Care, or Disability

  • A strong understanding of Resident Rights and Aged Care Accreditation Standards
