District Nurse

We are seeking a highly motivated and experienced Registered Nurse to join our District Nursing team in a part-time capacity. As a District Nurse, you will provide high-quality nursing care to a diverse group of clients, utilizing a problem-solving approach to assessment, planning, implementation, and evaluation.

• Provide quality nursing care to clients in their homes and in the community
• Use a problem-solving approach to assessment, planning, implementation, and evaluation
• Provide health education to clients and their carers about their identified health needs
• Liaise closely with community service providers to coordinate client care
• Act as client advocate to assist positive health outcomes
