LPN Hospice Crisis Care Specialist
We are seeking a compassionate and skilled LPN to provide patient care in a hospice crisis setting.
- Responsibilities:
- Deliver high-quality direct patient care as defined by the State Nurse Practice Act, ensuring accurate and timely documentation of patient services.
- Implement current nursing practice following comprehensive assessments and plans of care.
- Assess and educate patients and families on self-care plans, providing information pertinent to diagnosis and treatment.
- Collaborate with the interdisciplinary team to coordinate hospice services and identify patient needs.
- Manage pain and symptoms effectively, evaluating patient responses to treatments and medications.
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings.
- Current licensure as an LPN/LVN in the practicing state.
- CPR certification is required.
- Excellent communication, organization, and time management skills are essential.
- A valid driver's license and insured automobile are necessary for travel.
- Familiarity with the philosophy of hospice care and the needs of terminally ill patients is expected.
