About the Role
:We are seeking an experienced clinician in nursing (RN) to join our team as a Healthcare Navigator for Acute Patient Care. In this role, you will utilise your clinical skills and people skills to manage acute patient care, enhancing and promoting their quality of life.
This is a busy service that demands the best talent to help us lead and deliver exceptional patient outcomes. You will work across a 7-day roster between the hours of 7am and 10pm, with opportunities for full-time and part-time employment. Our Virtual Care Centre is based in Eastwood, Adelaide, offering flexible working options including remote work from home.
Your Key Responsibilities
You will be responsible for managing patient's acute care health requirements in a professional manner, utilising virtual technology and partnering with our home visiting provider team. Your key responsibilities will include:
- Establishing, monitoring and coordinating patients' treatment plans.
- Applying your clinical skills to think on your feet and problem solve to ensure best practice care for our patients.
- Working collaboratively with multidisciplinary teams in acute healthcare settings.

About You
To succeed in this role, you will require:
- An Australian Registered Nurse qualification (RN).
- Experience in acute care management, preferably in a hospital or community setting.
- Strong clinical reasoning and assessment skills.
- Excellent communication and interpersonal skills.
