The position of Aboriginal Social Worker exists within the Nunyara Aboriginal Health Unit, as a part of the local health district.
This role is responsible for delivering and monitoring social work services to clients referred to the Aboriginal Chronic Care service. Referrals are from the acute and community sectors for clients with acute or chronic conditions, and their carers, requiring social work support, assessment and intervention.
Key Responsibilities:
- Provide professional effective social work service to patients and their family/carers to ensure the service is culturally safe.
- Assist our Aboriginal and Torres Strait Islander community to deal with the emotional impact of illness through assessment and counselling.
- Undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services.
- Work closely with the whole Aboriginal Health Unit and a range of other professions, support services in hospital and community to ensure the development and delivery of effective and proactive social work services.
The successful applicant will have:
- A degree in social work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (overseas qualifications must supply the AASW Overseas Assessment number).
- Unencumbered driver's license and ability to travel across the District and to other external sites as required.
