- Coordinate Care Services: Oversee the implementation of care plans, ensuring they are consistently followed and adjusted as necessary.
- Monitor Resident Health and Well-being: Monitor residents' health and well-being, intervening promptly to address any changes or concerns.
- Facilitate Communication: Act as a liaison between residents, families, staff, and external healthcare providers to facilitate seamless communication and continuity of care.
- Manage Staffing and Performance: Manage staffing levels, rostering, and performance to ensure adequate and skilled care provision.
- Ensure Compliance: Ensure compliance with regulatory standards and organisational policies.
- Foster a Supportive Environment: Foster a supportive and respectful environment that promotes the dignity, independence, and safety of residents.
- Qualifications: Bachelor's degree in nursing, or a related field (preferred).
