Key Responsibilities:

  • Conduct client assessments and develop personalized care plans to meet their unique needs
  • Provide high-quality nursing care and in-home services as needed
  • Collaborate with clients, families, and healthcare professionals to ensure seamless care delivery
  • Manage client information, documentation, and billing in accordance with company policies and regulations
  • Ensure compliance with local, state, and federal regulations related to aged and disability care services

Requirements:

  • RN in community nursing with current AHPRA registration
  • Minimum 1-2 years of experience in community nursing
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • High level of computer literacy and proficiency in relevant software

Preferred Skills:

  • Experience in aged care, disability, or mental health services
  • Knowledge of clinical governance and continuous improvement principles
  • Ability to problem-solve and think critically in a fast-paced environment
