About the Role:

Australian Unity is seeking an experienced and caring individual to join our team as a Home Care Support Worker, providing support services to our aged care customers to help them live independently and remain within their own homes.

Key Responsibilities:

  • Assist customers with activities of daily living tasks based on care plans, including personal care, domestic assistance, social support, and administration.
  • Provide transportation to appointments and social outings, shopping, and emotional support and companionship to customers.
  • Report and document progress notes to ensure continuity of care.
  • Work collaboratively with customers, families, and other healthcare professionals to deliver high-quality care.

Requirements:

  • 12 months+ experience and/or completed a Cert III in Individual Support / Disability / completed 1st year of a Nursing degree.
  • Flexibility to work Monday to Friday within the hours of 7am - 6pm.
  • Current Australian Driver's Licence and ownership of a reliable insured vehicle.
  • Fully vaccinated (2) for Covid-19.
  • Ability to commit to our PAID 2 week Brilliant@Basics training and induction program when you first commence.
