Job Overview

The Clinical Aged Care Assessor role involves conducting comprehensive assessments to evaluate and determine the needs of customers seeking home support services.

You will develop personalized support plans and assist in connecting relevant service providers, enabling customers to maintain their independence.

Key Responsibilities:
  • Collaborate with customers, carers, families, representatives, and other service providers throughout the aged care planning process.
  • Undertake and review personalized support plans, effectively managing changing customer and carer needs with their consent.
  • Identify customers' formal and/or informal respite support arrangements.
  • Develop an Emergency Care Plan if the customer does not have one in place.
  • Refer for services, assess complexity, and risk of vulnerability, and assist with linking support, reablement, and short-term case management.
Requirements:
  • Possess a tertiary qualification in a health-related discipline, such as a Registered Nurse, Physiotherapist, or Social Worker.
  • Hold current unrestricted AHPRA registration or be eligible to join a relevant professional association.
  • Demonstrate at least one year of experience delivering services in aged care settings and/or to aged persons.
  • Have knowledge or experience conducting home assessments or risk assessments for vulnerable individuals.
  • Be able to communicate clearly and effectively with customers who may have hearing loss, sensory loss, or cognitive impairment.
