Female Mental Health Support Worker Wanted

Experienced Female Mental Health Support Workers in Toowoomba

We are currently looking for a compassionate and skilled Female SW with fluent English, extremely reliable, to join our team to provide quality support to our participants in Toowoomba.

About the Role:

  • Provide high-quality support to participants which ranges in their level of needs
  • Adherence to service agreement requirements of the funded package & meeting of deadlines
  • Keep accurate and up-to-date documentation
  • Work independently within the community or the participant's home
  • Reporting after each shift
  • Support participants to achieve positive outcomes of their personal goals as stated within their NDIS plan
  • Transport participants to a range of different services and appointments
  • Participate in corporate and community events
  • Monitor participant's health and well-being, whilst also contributing to their skill development

Key Responsibilities:

  1. Assisting participants with daily activities, transport and community access
  2. Ensure care plans and other information are followed and any changes are escalated to clinical leads efficiently and safely
  3. Using complex care equipment and carrying out complex care procedures
  4. Supporting clients outside the home when required i.e medical appointments
  5. Creating a safe and supportive environment that fosters dignity, respect, and comfort
  6. Providing emotional support and companionship to participants and their families
  7. Conducting sleepover shifts and day shifts as scheduled to ensure continuous care
  8. Ensuring participants' physical, emotional, and psychological well-being

Requirements:

  • Minimum 4-5 weekdays of availability for day, evening and sleepover shifts
  • Nationwide Police Certificate (NPC)
  • NDIS Worker Screening Clearance (Yellow Card)
  • Valid CPR and First Aid Certificate
  • Current Driver's License and reliable motor vehicle
