Job Summary

We are seeking a skilled and compassionate Care Finder and Support Worker to join our team at Kirinari Community Services. As an integral part of our team, you will play a vital role in providing specialist and intensive assistance to help individuals understand and access aged care services and connect with relevant supports in the community.

About the Role

This is a full-time position that requires a strong understanding of the ageing process, related life transitions, and the various aged care supports and services available in the local and regional communities. You will work closely with clients who are isolated or have communication barriers, and provide assertive outreach to engage and build rapport with potential clients and local intermediaries.

Your primary responsibility will be to support the needs of people with diverse backgrounds and life experiences, treating them with dignity and respect, and in a way that values their identity, diverse backgrounds, and life experiences.

Key Responsibilities
  • Provide specialist and intensive assistance to help individuals understand and access aged care services and connect with relevant supports in the community.
  • Support improved integration between health, aged care, and other systems at the local level.
  • Engage and build rapport with potential clients and local intermediaries, including outreach activities to target groups and locations.
  • Work with clients who are isolated or have communication barriers, and provide assertive outreach to engage them.
  • Support the needs of people with diverse backgrounds and life experiences, treating them with dignity and respect.
Requirements
  • Relevant qualifications (e.g. social work, human services, aged care, community services or health) and/or relevant experience.
  • Demonstrated understanding and working knowledge of the ageing process and related life transitions.
  • Demonstrated understanding of aged care supports and services available in the local and regional communities and process and eligibility requirements to access.
  • Commitment to delivering a person-centred approach that respects and responds to each person's individual needs, preferences, values, and life experiences.
  • Well-developed communication and interpersonal skills and ability to engage and communicate effectively with people from a culturally and linguistically diverse (CALD) background and Indigenous communities.
