Job Title: Hospice Nurse
Job Summary:Our organization seeks a compassionate and skilled Hospice Nurse to provide professional nursing care to patients with life-limiting illnesses. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work collaboratively with other healthcare professionals.
Responsibilities:- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Develop and implement individualized care plans that address patients' unique needs and goals.
- Provide skilled nursing care, including administering medications, wound care, and pain management.
- Collaborate with other healthcare professionals to ensure seamless transitions in patient care.
- Current licensure as a Registered Nurse (RN) in good standing.
- Minimum one year general nursing experience, preferably in hospice or palliative care.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively as part of a team.
- Experience working with electronic medical records.
- Basic Life Support (BLS) certification.
- Certification in Hospice and Palliative Nursing (CHPN).
