A Homelessness Case Manager will provide case management support to individuals who are homeless or at risk of homelessness.


Key Responsibilities
  • Work closely with participants to develop, review and implement case management plans tailored to their individual needs.
  • Offer practical and personal support, crisis budgeting, information, advice and community referrals.
  • Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as necessary.
  • Provide support that is proactive, persistent, reliable, practical and comprehensive in a manner that respects the dignity and privacy of individuals.
  • Enable participants to maximize their capacity for independent living.
  • Advocate and assist participants to access housing and specialist services, such as drug or alcohol treatment, mental health services, legal assistance and support for domestic and family violence.

Essential Requirements
  • Certificate IV qualifications in Community Services or a related field.
  • At least 2 years of relevant experience.
