Job Description

We are seeking a dedicated individual to coordinate, care plan and monitor clients under the Community Care Package (CHSP) program. As a CHSP Coordinator, you will work autonomously as part of a multi-disciplinary team and build strong relationships with clients and their families.

This role requires effective communication, problem-solving and organisational skills, ability to prioritise and multi-task, be self-motivated with a 'can do' attitude, and possess good computer skills including care databases, Outlook, MS Word and Excel.

The ideal candidate will have experience as a CHSP or Home Care Packager Coordinator, an ability to speak another language is highly regarded, and be a permanent Australian resident with a National Police Check (or willingness to obtain), open Australian Drivers Licence, and be fully vaccinated - COVID19 and Influenza.

Key Responsibilities:

  • Coordinate, care plan and monitor CHSP clients to ensure quality services are provided.
  • Work closely with direct care workers, providing guidance where needed.
  • Build good relationships with clients and their families.
  • Ensure all care plans meet client needs and preferences.
  • Communicate effectively with multidisciplinary teams.

Requirements:

  • Experience as a CHSP or Home Care Packager Coordinator.
