Job Title: Advanced Practitioner

We are seeking a skilled and experienced professional to join our team as an Advanced Practitioner.

This is a challenging and rewarding role that requires strong case management skills, effective communication, and the ability to work with complex clients.

  • Key Responsibilities:
  • Conduct risk assessments and develop strategies to mitigate risks.
  • Prepare case plans and provide interventions to support clients in achieving their goals.
  • Coordinate service providers and care teams to ensure comprehensive support for clients.
  • Address the underlying causes of offending behavior and develop strategies to prevent reoffending.
  • Lead and develop case management staff to achieve best practice outcomes.
Required Skills and Qualifications:
  • Demonstrated experience in effective case management practices with complex clients.
  • Excellent verbal communication skills and confidence in dealing with people exhibiting challenging behaviors.
  • A proven ability to model pro-social behaviors to a caseload of individuals.
  • A passion for supporting clients in the justice system while contributing to community safety.
  • An ability to lead and mentor staff to achieve their best.
