As a trusted advisor, you will support and empower clients to make informed decisions about their home-based care.
Key Responsibilities:- Conduct comprehensive wellbeing assessments to develop individualised plans addressing client goals and maximising quality of life.
- Partner with clients, families and multidisciplinary teams to respond to changing needs and ensure timely updates to stakeholders.
- Manage client feedback, administrative tasks and caseload to ensure smooth operations.
We seek a committed and compassionate individual who is sensitive to diverse client needs, adaptable to rapid changes and proactive in continuous learning and problem-solving.
Essential Criteria:- Relevant tertiary qualifications in Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields.
- Experience in client case management.
- National Police check.
- Driver's license.
