About the Role

This is a hybrid flexible role, Monday to Friday 9:00 am to 5:00 pm and will deliver support coordination services working from home (WFH) and one day a week at our head office in South-west Sydney.

You will support participants to understand their NDIS plan, goals and objectives while promoting choice and control and supported decision making.

Key Responsibilities
  1. Coordinate mainstream, informal and formal supports with participants to assist them in achieving their goals.
  2. Establish a respectful and collaborative working relationship with participants and their family/support network that encourages capacity building and promotes choice, control and inclusion.
  3. Develop strong and collaborative working relationships with relevant internal and external stakeholders including Communities and Justice, health, education, employment along with Community NFPs and other NDIS Providers to ensure effective communication, referrals and support coordination can occur.
  4. Assisting participants where there are points of crisis or escalation of needs.
  5. Regularly monitor NDIS plan funding to ensure services are being delivered within the participant's allocated budget.
  6. Engage with NDIA regional contacts, including Local Area Coordinators.
  7. Monitor and report progress against participant NDIS goal plans and budgets.
  8. Compliance with Circle of Hope's values, code of conduct, policies and procedures and relevant government legislation, standards, rules and guidelines where relevant.
  9. Maintain the privacy and confidentiality of participants, carers, families and the organisation.
Selection Criteria
  • Tertiary qualification and significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice, Child Protection, Psychology, Social Work, Employment Services).
  • A minimum of two (2) years' experience as a NDIS Support Coordinator, NDIS LAC or Planner is required.
  • Demonstrated ability to be client-focused and work collaboratively with participants, carers, loved ones and stakeholders.
  • Established time management, financial budgeting, problem solving and conflict management skills.
  • Ability to self-motivate, complete tasks within set time frames, manage caseload and conflicting deadlines and work within a Key Performance Indicator framework.
  • Strong written, administration and computer skills for the creation of case notes, reports, correspondence and other documents in a cloud-based management system.
  • Strong interpersonal skills with a positive attitude and a flexible and adaptive nature to respond to the changing needs of our clients.
  • Experienced with change of circumstances and NDIS plan review processes.
  • Applicants will be subject to a NDIS Worker Screening Check, Working with Children Check, and four (4) NDIS mandatory eLearning modules.
  • Current NSW Driver's License and willingness to drive own vehicle.
  • National Police Check
  • Right to work within Australia.
