About the PositionWe are seeking a dedicated and caring Disability Support Worker to provide support to individuals with significant physical and mental health needs, helping them live more independently.
Daily Responsibilities:
- Assist with personal care, including bathing, dressing, and grooming
- Facilitate safe and structured transitions from hospital to home
- Deliver ongoing tailored support to meet the individual's needs
- Experience as a Support Worker and/or Dementia caregiver
- Excellent interpersonal skills and communication abilities
- Knowledge of safeguarding practices and protocols
- Compassionate and empathetic nature
- Ability to work effectively in a team and independently
