Job Title: Clinical Care Coordinator

We are seeking a skilled and compassionate Clinical Care Coordinator to play a key role in providing high-quality, person-centered clinical care to all residents within our medium-sized care service.

Key Responsibilities:
  • Deliver exceptional clinical care to residents, promoting their overall well-being and quality of life.
  • Maintain accurate and up-to-date resident documentation, ensuring seamless communication with the care team.
  • Create a safe and welcoming home environment for all residents, fostering a sense of community and belonging.
  • Collaborate with colleagues to ensure efficient and effective funding assessments and documentation processes.
  • Mentor, train, and lead care team members, driving excellence in clinical practice and teamwork.
Requirements:
  • AHPRA RN registration with Australian aged care experience, demonstrating a strong commitment to delivering high-quality care.
  • In-depth understanding of aged care standards and quality indicators, ensuring compliance and excellence in practice.
  • Exceptional verbal and written communication skills, facilitating effective collaboration and information sharing.
  • Familiarity with AN-ACC funding systems, enabling informed decision-making and strategic planning.
