About the Role
The District Nurse will provide high-quality nursing services, including assessment and care planning, to a diverse community. This role requires a wellbeing and reablement approach to improve the health status and independence of clients and their carers.
Key Responsibilities
- Provide high-quality nursing services within professional boundaries and responsibilities.
- Promote and maintain the active involvement of clients and their carers in care planning.
- Liaise and work co-operatively with local service providers to ensure seamless and appropriate delivery of services.
Requirements
- Relevant clinical experience within the community setting and/or across a broad range of acute medical/surgical services.
- Demonstrated highly effective interpersonal skills, including problem-solving, conflict resolution, negotiation, and teaching.
- Experience working with clients with multiple comorbidities and complex care needs.
- Excellent organisational, coordination, and time management skills.
- Current nursing registration with AHPRA.
- Current Driver's Licence.
- Current National criminal history check, or willingness to obtain, and Current NDIS Worker Screening Check.
